Graduate Certificate Completion Form

If you are a degree student that is completing a certificate, the certificate will not be issued until you graduate with your degree. Please list your degree graduation date, unless you are a certificate only student. A paper certificate is mailed approximately eight weeks after the end of the semester in which you graduate.
