Tips For Completing Initial Registration Form

Be sure to note the following听(bullets 3 and 4 may appear to diverge from the instructions but are essential):听

  • Be sure to use Form 225 for initial registration.
  • For the 鈥淪elect One Business Activity鈥 option, select 鈥淩esearcher.鈥
  • CU personnel are State employees, and are not required to pay the registration fee. 听Check the "exempt" box and indicate the following as the certifying institutional official: 听Dr. Jon Reuter, Associate Vice Chancellor of听Research Integrity, 99 UCB, 303-735-5809.听
  • The State of Colorado no longer issues state licenses to researchers, thus听unless you have a license to prescribe, leave Section 3听(State Licenses) blank.听The second entry in section 3听should also be left blank, since the State of Colorado does not issue State Controlled Substance Licenses.
